The schedule’s rightmost column is a daily maximum, not a unit rate — it is the unit price times the daily unit cap. Reading it as the rate would overstate 97153 by 32 times.
Each code is priced in exactly one provider tier: Level 1 is BCBAs and certified developmental clinicians, Level 2 is registered behavior technicians and developmental technicians, Team is one BCBA plus two RBTs.
The schedule prints only "Updated 05/14/2026". The 15 January 2022 effective date comes from HFS’s own link label on the ABS services page.
Every ABA code requires prior authorization.
