The workbook carries a publication date of 07/28/2026 in cell A1, not an effective date. Per-row Rate Effective Dates range from 2022-07-01 to 2025-12-18. MHD states the schedules are updated monthly, so the file URL is stable but the contents move.
Provider tiers are carried in the modifier columns and are labeled in a Provider Type column: HO = Behavior Analyst or Psychologist, HN = Assistant Behavior Analyst, HM (as second modifier) = Registered Behavior Technician, TM = telemedicine, U8 = a small premium applied only to 0362T, 0373T and 97155 HO. Several codes also carry a Rural Health Clinic row with no modifier.
There is no unit column. Only 0362T and 0373T print "ea 15 min" in their descriptions. The workbook does show Maximum Units per code (97153 = 32, 97151 = 32, 97152 = 16, 97155 = 24, 97156 = 16, 97154 = 8, 97157 = 8, 97158 = 6), which is consistent with 15-minute units but is not a statement of one.
97154 at $2.05 and 97157/97158 at $2.14-$3.16 are group rates and are an order of magnitude below the individual codes. Do not present them alongside 97153 without saying so.
