Read before using these numbers

Two dates are printed on this document and they differ. The workbook's Code Key sheet says the fee schedule update was published 1 July 2026, but the ABA worksheet's own Rate Effective Date column prints 7/1/2024 against every ABA code. The effective date shown here is the per-row date, which is when the dollar amounts took effect; the 2026 file is a republication. Figures are unchanged from the February 2026 edition.
97158, 0362T and 0373T do not appear anywhere in the Behavioral Health Fee Schedule, nor in the 48,889-row main Oregon Health Plan fee schedule. Oregon publishes no rate for them.
Oregon prints the time unit on the schedule itself — 'Per 15 Minutes' in the Unit column for every code above — so no companion document is needed to read these rates.
This is the fee-for-service (open card) schedule. Coordinated Care Organizations, which cover the large majority of Oregon Health Plan members, are not bound by it for contracted providers, though they must pay these rates to non-contracted providers. CCO contracted rates are not public.
Two modifiers change what is actually paid: TN (culturally and linguistically specific services, rural) reimburses at 27 percent of the fee-for-service rate and U9 (non-rural) at 22 percent. GT is the telehealth modifier and is the only modifier the schedule lists as allowable on the ABA codes.
Cells store unrounded values such as 20.84544, but the schedule's own number format displays two decimals. The displayed values are what is published here.
OAR 410-172-0760 governs ABA provider qualifications and assessment requirements but contains no codes, units or rates.
